Online Internet Services

We have for some time provided for patients, who register for online services, to order repeat prescriptions and book doctor appointments online. From the 20th March 2015 we are enhancing the online services by providing patient’s access to their Medical Records (initially Medication, Immunisations records, Allergies and details of any adverse reactions).

Patients requiring access to these services have to complete a “Patient Online: Registration Form”, obtainable either from the surgery or you can download a printable copy here or complete the form online from this link here here, print and bring the completed form with you to the surgery. As well as the registration form you must bring with you proof of identity which should include photo identity, such as Passport, photo Driving License or Firearms certificate. In addition we also require proof of address, such a Council Tax statement, Utility bill, Bank/building society statement, Mortgage statement, benefit statement or work permit or visa all of which must have been issued within the last 3 months.

Eagle House Surgery

Eagle House Surgery is based in a Georgian building, situated in White Cliff Mill Street, Blandford  (150 yards from road fork on the left hand side).  Parking is available behind the surgery premises in a pay and display car park. The Families’ Medical Centre, for patients registered at Blandford Camp, is near the shop, next to the Men’s Medical Centre. 

Both Surgeries are accessible to disabled visitors; however, if you require help entering the surgery, please contact the receptionist who will be pleased to assist.

Existing patients can enjoy access to our wide range of services at the touch of a button. Whether managing your appointment or ordering a repeat prescription you can now do both without leaving your home, by registering for Online Internet Services.
